    AFFIDAVIT OF SERVICE

    I, GENALYN B. UJANO, married, of legal age, Filipino Citizen, staff of


    the City Legal Services Office of the City Government of Vigan, and with post-
    office address at Burgos St. corner Quezon Avenue., Barangay I, Vigan City,
    Ilocos Sur after being duly sworn to in accordance with law, hereby depose and
    state:
    1. That on April 27, 2017, I served a copy of a Petition for Review,
    addressed to the following parties by registered mail in accordance with Section 3
    and 5 in relation to Section 10 Rule 13, by depositing a copy in the post office, in a
    sealed envelope fully prepaid, as evidenced by Registry Receipt Nos. with
    instructions to the postmaster to return the mail to the sender after ten (10) days, if
    undelivered:

    not practicable due to the distance and lack of manpower and logistics, thus service
    via registered mail is resorted to.
