Alternate Contacts

The Alternate Contact information is used to attempt contact with a job seeker when they cannot be contacted using the information in Contact Information. Alternate Contact screenshot

Add Alternate Contact

  1. Select Alternate Contacts from the Contact Info section of Case Details. The Alternate Contacts page displays.
  2. Select Add Contact. The Add an Alternate Contact page displays. Add Alternate Contact screenshot
  3. At minimum, complete all required information, which is indicated by (required).

    FieldDescription
    First Name (required)(Text) Enter the contact's first name.
    Middle Initial(Text) Enter the contact's middle name or initial.
    Last Name (required)(Text) Enter the contact's last name.
    Relationship to Participant (required)(Drop-down) Select from the following options:
    • Parent
    • Sibling
    • Extended biological family member
    • Partner
    • Friend/social support network member
    • Other
    Relationship, if Other selected above(Text) Enter a description of the relationship between the contact and the job seeker.
    Address Line 1 (required)(Text) Enter the contact's street address.
    Address Line 2(Text) Enter the contact's additional address info, such as apartment or suite number.
    ZIP (required)(Digits) Enter the contact's ZIP. The City and State are automatically populated once a selected ZIP is selected or when you tab/click away from this field.
    City (required)(Text) Populated from the ZIP code but can be modified if needed.
    State (required)(Drop-down) Populated from the ZIP code but can be changed if needed. Defaults to the application's state.
    Country (required)(Drop-down) Select the contact's country. Defaults to the United States.
    Email(Text) Enter the contact's email.
    Work Email(Text) Enter the contact's work email.
    Phone(Digits) Enter the contact's primary phone.
    Alternate Phone(Digits) Enter another phone number for the contact.
    Work Phone(Digits) Enter the contact's work phone number.

  4. Select Save Contact. The Alternate Contacts page displays with the new entry. Select Cancel to return to the Alternate Contacts page without saving.

View/Edit/Delete Alternate Contacts

  1. Select Add Contact. The Add an Alternate Contact page displays.
  2. To view or edit an existing contact, select Edit. The Edit Alternate Contact page displays.
  3. Update the necessary information and select Save Contact
  4. To remove the contact, select the Delete trashcan. A pop-up displays, requesting confirmation. Select Delete to remove the record. The Alternate Contacts page displays.
