Uses of Enzastar 500

Indication
In combination with pembrolizumab (Keytruda) and platinum chemotherapy for the initial treatment of patients with metastatic non-small cell lung cancer (NSCLC) without EGFR or ALK genomic tumor aberrations . In combination with cisplatin in the initial treatment of patients with locally advanced or metastatic non-small cell lung cancer (NSCLC). Indicated as a single agent for the maintenance treatment of patients with metastatic or non-small cell lung cancer (NSCLC) whose disease has not progressed after four cycles of platinum-based first-line chemotherapy . Indicated as a single agent for the treatment of patients with recurrent metastatic non-small cell lung cancer (NSCLC) following prior chemotherapy. Limitations of Use: Alimta is not indicated for the treatment of patients with non-squamous cell lung cancer.
Indicated, in combination with cisplatin, for the initial treatment of patients with malignant pleural mesothelioma (MPM) whose disease is incurable or who are otherwise not candidates for curative surgery.
Dosage - How to use
Take a corticosteroid (equivalent to dexamethasone) 4mg x 2 times/day the day before, the day of infusion and the day after the infusion of pemetrexed. Add at least 5 doses of folic acid (or a multivitamin containing folic acid (350-1000 mcg)) for 7 days before the first dose of pemetrexed, continue during the course of treatment, and for 21 days after the last dose of pemetrexed. Inject vitamin B12 (1000 mcg) IM in the week before the first dose of pemetrexed and every 3 cycles thereafter (possibly on the same day as pemetrexed).
Cisplatin combination: First day of each 21-day cycle: 500 mg/m2 ALIMTA intravenous infusion over 10 minutes, about 30 minutes later, 75 mg/m2 infusion over 2 hours. Antiemetic and rehydration before and/or after cisplatin infusion.
Single use: Non-small cell lung cancer with chemotherapy: First day of every 21-day cycle: 500 mg/m2 intravenous infusion over 10 minutes. Dose adjustments for subsequent cycles are based on toxicity, in addition to hematologic and neurotoxicity.

Side effects:
Neutropenia/granulocytosis, leukopenia, Hb reduction, thrombocytopenia. Conjunctivitis . Diarrhea, vomiting, stomatitis/throat, nausea, loss of appetite, constipation, dyspepsia. Tired. Loss of water. Sensory neuropathy. Taste disorder. Increase creatinine, decrease ClCr. Ban, hair loss. Contraindications
Subjects should not take Enzastar 500
Hypersensitivity to drug ingredients. Discontinue breast-feeding during treatment with pemetrexed. Concomitant use of yellow fever vaccine. Notes/ Precautions
Precautions before taking Enzastar 500
ClCr < 45mL/min, children and adolescents < 18 years old: not recommended. Patients with dehydration, hypertension, diabetes mellitus, pre-existing cardiovascular risk factors; Clinically significant third compartment fluid; Adult males (may cause genetic damage); Radiation treatment; Mild to moderate renal failure; Pregnant; Driving/operating machines. FDA Classification in Pregnancy Level D: There is evidence of a risk in the human fetus, but because of the potential benefit, its use during pregnancy may be approved, regardless of the risk (as needed) medication is necessary in life-threatening situations or in a serious illness for which safe medications cannot be used or are ineffective).

When using two or more drugs at the same time, drug interactions often occur, leading to antagonism or synergism.
